ACHIEVE LACROSSE, LLC
CONSENT TO PLAY, WAIVER AND LIABILITY RELEASE
In consideration of being allowed to participate in an Achieve Lacrosse, LLC program, tournament, league, camp, training session, clinic or any other event (individually and collectively an "Event"), including any such Event being operated, advertised or organized under the name of Mass Elite, the undersigned acknowledges, certifies and agrees as follows:
I/my child am/is voluntarily choosing to participate in an Event and acknowledge that lacrosse is a high-speed sport which will involve some contact that may result in injury, concussion, death or damages. I/my child am/is in good physical condition necessary to participate in the Event and am aware of no medical conditions, illnesses, impairments, infirmities or injuries that would interfere with or prevent me/my child from participating in all aspects of an Event.
I understand that each participant at an Event will be engaging in activities that by the inherent nature of the activity involves a risk of serious injury, including the potential of permanent disability and death, and understand and accept that severe loss of any kind may result from my/my child's own actions and conduct as well as the actions and conduct of each participant, the rules of play, and/or from any equipment at the facility where an Event is taking place, whether or not such equipment is used in the normal course of play and assume all risks associated with the same.
I will inspect the property or facilities at which an Event is being held prior to my participation and if I observe or believe anything to be unsafe, I will immediately advise Achieve Lacrosse, LLC directors or Event managers of such conditions.
I acknowledge my participation includes possible exposure to and illness from infectious diseases, including but not limited to MRSA, influenza, and COVID-19. I understand that any pre-existing health conditions may lead to greater risks due to exposure to COVID-19 or any other infectious diseases. I knowingly and willingly agree to comply with all recommendations provided by federal, state and local government concerning infectious diseases, including but not limited to, COVID-19.
I am aware of the information concerning concussions and the protocol in addressing, treating and managing concussions as outlined in the CDC Heads Up: Concussion in Youth Sports Program, https://www.cdc.gov/headsup/youthsports/index.html. If at anytime during my/my child's participation in an Event, I experience or observe my child experience any sign or symptoms of a concussion, I agree to remove myself/ my child from continued participation in the Event and shall not return to participate in such Event without the consent and clearance from appropriate medical professionals.
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, and assume full responsibility for my participation in an Achieve Lacrosse, LLC Event and hereby waive, release and forever discharge Achieve Lacrosse, LLC, and the Releasees, as hereinafter defined, from any and all responsibilities or liability for all injuries, illnesses, death, damages, disabilities, causes of actions, losses or claims for personal injuries or property damage, resulting from my participation in or attendance at an Event to the fullest extent permitted by law.
I hereby consent to and agree to Achieve Lacrosse, LLC, including without limitation, its directors, coaches, agents, trainers, referees and medical staff, providing any and all medical care or other medical assistance as may be deemed necessary to me/my child, and also my/my child's removal from continued play and making arrangements for transportation to a medical facility, in the course of my/her participation in an Event.
I agree to indemnify, defend and hold harmless Achieve Lacrosse, LLC, lessors and/or owners of premises used to conduct any Event, and any officer, shareholder, member, director, coach, employee, agent, contractor, affiliate, or volunteer in any capacity for these organizations (individually and collectively the "Releasees"), for any and all injuries, illnesses, damages, disabilities, demands, causes of actions, losses or claims for personal injuries, death or property damage, arising from my/my child's participation at an Event.
FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION) This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this CONSENT TO PLAY, WAIVER AND LIABILITY RELEASE to my child/ward including the risks of presence and participation and her personal responsibilities for adhering to the rules and regulations for protection against diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I for myself, my spouse, and child/ward do consent and agree to her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify, defend and hold harmless the Releasees for any and all injuries, illnesses, damages, disabilities, causes of actions or claims for personal injuries or property damage, incident to or resulting from my minor child's presence or participation in an Event to the fullest extent provided by law.
IMAGE RELEASE

I give permission to Achieve Lacrosse, LLC and its agents to use photograph(s) taken of my daughter(s) for specific use on its web site and in promotional materials. I understand the photograph(s) will be used in all forms of media including, but not limited to, digital media, social media, portfolios, articles and promotional material for Achieve Lacrosse, LLC . I give permission for my daughter's name to be used in conjunction with the photographs. I hereby have the right at any time to withdraw the permission granted herein by providing written notice of said withdrawal to the Achieve Lacrosse, LLC organization. In giving my consent, I hereby release and hold harmless Achieve Lacrosse, LLC and its agents from any and all responsibility or liability in connection with the use of such images.
I have read, fully understand, accept and agree to all of the terms set forth in the Achieve Lacrosse, LLC Consent to Play, Waiver and Liability Release.
